Resumo: A hipotermia trata-se de uma alteração fisiológica recorrente em ambiente cirúrgico que ocorre geralmente após a indução anestésica promovendo a diminuição do metabolismo e a inibição da termorregulação. O enfermeiro pode avaliar a Hipotermia e identificá-la como diagnóstico de enfermagem (DE), sendo fundamental que a taxonomia seja revisada e avaliada em diferentes populações. O objetivo desse estudo foi validar o DE Hipotermia (00006) em pacientes adultos no perioperatório de cirurgias eletivas em Centro Cirúrgico. Trata-se de estudo de validação de DE realizado em três etapas: construção das definições conceitual e operacional dos elementos do referido DE; análise de conteúdo por especialistas e validação clínica. A primeira etapa foi realizada por meio de Revisão Integrativa, permitindo a construção das definições conceituais e operacionais dos elementos do referido DE, sendo subsidiada por 40 artigos. A segunda etapa, Análise de conteúdo, foi realizada por 50 especialistas, classificados segundo critérios de experiência clínico-cirúrgica, ensino e pesquisa na área em questão. Para tal, foi utilizada uma escala tipo Likert de cinco pontos, considerando cada elemento do referido DE desde nada característico a muito característico do DE para a população, sendo aplicado o teste binomial, com proporção de concordância entre os especialistas maior ou igual a 85%. A idade média dos especialistas foi de 38,14 anos, a maioria especialista em CC, seguidos por mestres, doutores e com publicações envolvendo a temática explorada. O tempo de formação média foi de 13,64 anos e experiência em CC de 8,82 anos. As características definidoras (CD) validadas foram: baixa temperatura corporal acidental, baixa temperatura corporal acidental em pacientes adultos com lesão, calafrios, pele fria ao toque, piloereção e vasoconstrição periférica. E, os Fatores relacionados (FR) foram a baixa temperatura ambiental e conhecimento insuficiente do cuidador sobre a prevenção da hipotermia. Por fim, foi realizada a validação clínica por meio de estudo longitudinal utilizando medidas de acurácia e métodos avançados para validação. Nessa fase, foi utilizada sensibilidade, especificidade e valor preditivo positivo e negativo para identificar as CD com melhor poder de predição do DE. Os dados foram coletados por meio de Histórico de enfermagem contendo variáveis sociodemográficas, anestésico-cirúrgicos e elementos do referido DE, avaliação do paciente, incluindo a mensuração da temperatura corporal. Participaram 255 pacientes adultos, de ambos os sexos, submetidos a cirurgias eletivas com duração acima de 60 minutos e excluídos aqueles com hipotermia induzida. A maioria era do sexo masculino, com idade média de 50,30 anos; 82,7% apresentou hipotermia e dentre esses, 93,8 % apresentou o DE Hipotermia. Foram validadas e consideradas preditoras do referido DE as CD Baixa temperatura corporal acidental, Baixa temperatura corporal em pacientes adultos com lesão, Pele fria ao toque, calafrios, Vasoconstrição periférica e Tremor pelo frio e ainda os FR Roupas insuficientes, Transferência excessiva de calor por condução, convecção e irradiação, Agente farmacológico, Suprimento insuficiente de gordura subcutânea, desnutrição. Abstract: Hypothermia is a recurrent physiological alteration in surgical environment that occurs generally after anaesthetic induction causing a drop in metabolism and thermoregulation inhibition. The nurse may measure and identify it as nursing diagnosis (ND), and it is essencial that the taxonomy be reviewed and evaluated in different populations. The goal of this study was to evaluate the nursing Hypothermia diagnosis (00006) in elective preoperative adult patients in the surgery center. This is a nursing diagnostic validation study carried out in three stages: building up definitions for both conceptual and operational elements of the referred (ND), content analysis from specialists and clinical validation. The first step was carried out through Integrative Review (IR), enabling the construction of conceptual and operational definitions of the elements of the ND, being subsidized by 40 articles. The second step, content analysis, was carried out by 50 specialists, classified according to clinical-surgical experience criteria, teaching and research in the area that evaluated the content. For this, a five-point Likert scale was used, considering each element of the referred (ND) from nothing characteristic to very characteristic of the diagnosis for the population, the binomial test being applied, with a proportion of accordance among specialists greater than or equal to 85%. The average age of the specialist ranged from 38 to 14 years of age, most of them holding a surgical center specialist title, assisted by masters, doctors and with publications regarding the giving topics. The qualification average time ranged from 13,64 and 8,82 years of experience in surgery center. The defining characteristics validated were: low accidental body temperature, low accidental body temperature in adult patients with injury, chills, cold skin when touched, piloerection and peripheral vasoconstriction. And, the related factors were low environmental temperature and caretaker’s insufficient knowledge on hypothermia prevention. Lastly, the clinical validation was carried out through a longitudinal study by using accuracy measures and advanced methods for evaluation. In this stage were used sensitiveness, specificity and positive and negative predictive value to identify the defining characteristic with stronger power of prediction of ND. The data were collected through nursing history containing sociodemographic variables, anesthetic-surgical procedure and elements of the referred (ND), patient evaluation, including body temperature measurement. 225 adult patients participated, both genders, submitted to elective surgeries lasting more than 60 minutes and those with induced hypothermia were excluded. The majority was made up of males, with average age of 50,30; 82,7% showed hypothermia and among the hypothermic ones 93,8% showed (ND) Hipotermia. The defining characteristics of the referred ND considered predictive and validated were hypertension, slow capillary refill, increase in oxygen consumption, and cold shiver and, even, related factors such as lack of clothes, excessive heat transfer by conduction, convection, evaporation and irradiation, pharmacologic agent, insufficient supply of subcutaneous fat and malnutrition.
