Resumo: A infiltração mandibular com o uso de articaína 4% têm obtido destaque em procedimentos realizados na mandíbula, sendo utilizada como técnica complementar ou como técnica de primeira escolha. Não há um consenso sobre o comportamento e mecanismo das infiltrações mandibulares com articaína frente ao sucesso e tempo de anestesia pulpar. Sendo assim, este trabalho tem como objetivo, através da TCFC, observar e dimensionar variações anatômicas na mandíbula que possam explicar estes questionamentos nos 1º e 2º molares e pré-molares. Para isso, foram selecionados 33 voluntários na Faculdade de Odontologia de Piracicaba – UNICAMP, que tinham indicação cirúrgica para exodontia de terceiros molares inferiores e necessidade de tomografia para melhor planejamento cirúrgico, minimizando os riscos associados. Além disso, todos os voluntários possuíam 1º e 2º molares e pré-molares inferiores hígidos e assintomáticos. Foi solicitado ao voluntário o exame TCFC, e em seguida realizada uma sessão onde foi realizada a injeção utilizando um aparelho injetor computadorizado de anestésicos Morpheus. O ponto de punção foi entre as raízes no fundo de sulco adjacente à vestibular do primeiro molar inferior com um volume de solução anestésica de 1,8 ml da solução de articaína 4% com epinefrina 1:100.000. Antes e após a anestesia, foi realizado o teste elétrico com o auxílio do pulp tester e monitoramento dos sinais vitais. Após a anestesia, foi aguardado 1 minuto, e os testes no segundo minuto e em sequência, de 3 em 3 minutos foram realizados. Como critério de sucesso foi observado no pulp tester duas leituras consecutivas de 80 ?A e os testes elétricos foram repetidos em um ciclo total de 60 minutos. Foi considerado insucesso da anestesia se não obtida em até 10 minutos. Os pacientes foram orientados a preencher escalas analógicas visuais (EAV) para demonstrar experiências de dor/desconforto durante a aplicação da anestesia para inserção e penetração da agulha e injeção da solução e para o controle pós-operatório (24 horas). E dados referentes a intensidade de dor durante nos momentos citados e a correlação destes com período do ciclo menstrual das voluntárias testadas obtidos como resultados secundários. Dados do sucesso e tempo de anestesia foram correlacionados com a análise de características anatômicas na TCFC do grupo de dentes testados: largura da cortical óssea vestibular, distância dos ápices até a cortical óssea vestibular, distância dos ápices das raízes dos molares até o canal mandibular, largura, altura e tipo do forame mentual e distância do forame mentual até o ponto de aplicação da anestesia. Como resultado observou-se que a única correlação que se explica pelas características anatômicas analisadas é para 1º e 2º molares em associação com a largura da cortical óssea e a distância dos ápices das raízes destes dentes em relação a cortical óssea. Concluiu-se que quanto maior a largura da cortical óssea e a distância do ápice das raízes dos 1º e 2º molares à cortical, menor o tempo de duração da anestesia pulpar com articaína 4% com epinefrina 1:100.000 aplicada com a técnica infiltrativa mandibular

Abstract: Mandibular infiltration with the use of 4% articaine has been highlighted in procedures performed in the mandible, being used as a complementary technique or as a technique of first choice. There is no consensus on the behavior and mechanism of mandibular infiltrations with articaine in relation to the success and time of pulp anesthesia. Therefore, this work aims, through CBCT, to observe and dimension anatomical variations in the mandible that can explain these questions in the 1st and 2nd molars and premolars. For this, 33 volunteers were selected at the Faculty of Dentistry of Piracicaba - UNICAMP, who had surgical indication for lower third molar extraction and need tomography for better surgical planning, minimizing the associated risks. In addition, all volunteers had healthy and asymptomatic 1st and 2nd molars and lower premolars. The CBCT exam was requested from the volunteer, and then a session was held where the injection was performed using a computerized Morpheus anesthetic injector device. The puncture point was between the roots at the bottom of the sulcus adjacent to the buccal of the mandibular first molar with an anesthetic solution volume of 1.8 ml of the 4% articaine solution with 1:100,000 epinephrine. Before and after anesthesia, the electrical test was performed with the aid of the pulp tester and monitoring of vital signs. After anesthesia, 1 minute was waited, and the tests in the second minute and in sequence, every 3 minutes were performed. As a success criterion, two consecutive readings of 80 ?A were observed in the pulp tester and the electrical tests were repeated in a total cycle of 60 minutes. Anesthesia was considered failure if not obtained within 10 minutes. Patients were instructed to complete visual analogue scales (VAS) to demonstrate pain/discomfort experiences during anesthesia application for needle insertion and penetration and solution injection and for postoperative (24-hour) control. And data referring to the intensity of pain during the mentioned moments and the correlation of these with the menstrual cycle period of the tested volunteers obtained as secondary results. Data on success and duration of anesthesia were correlated with the analysis of anatomical features on CBCT of the group of teeth tested: width of the buccal cortical bone, distance from the apices to the buccal cortical bone, distance from the apices of the molar roots to the mandibular canal, width, height and type of the mental foramen and distance from the mental foramen to the point of application of anesthesia. As a result, it was observed that the only correlation explained by the anatomical characteristics analyzed is for 1st and 2nd molars in association with the width of the cortical bone and the distance of the apices of the roots of these teeth in relation to the cortical bone. It was concluded that the greater the width of the cortical bone and the distance from the apex of the roots of the 1st and 2nd molars to the cortical bone, the shorter the duration of pulpal anesthesia with 4% articaine with 1:100,000 epinephrine applied with the mandibular infiltrative technique
